Please download the CCSD Local School Agreement, click "enable editing," fill it out, and save it to your desktop. You will upload this form into the "DE Student Acknowledgement Form" under #26.

You must be logged into your CCSD computer (365 account)

for this to work.

You and your parent must initial next to each rule and sign this form for it to be complete. If you miss signing the last page, your application will be considered incomplete and you will NOT be offered a Friday DE meeting.

Next, click on the form below and answer the questions, acknowledge the rules and requirements, upload your testing scores for KSU or GA Tech (if applicable), and upload your initialed and signed

"CCSD Local School Agreement" in the form below.

To complete this form below, you must log in to your

Cobb County 365 account.

If you are interested in Chatt Tech, fill out the Chatt Tech form.

If you are interested in KSU, please fill out the KSU form.

If you are interested in Georgia Tech Distance Math, fill out the GA Tech form.

Do not fill out multiple forms.

If the form does not open,

you are not logged into your CCSD 365 account.

How to complete your
Dual Enrollment Funding:
Follow the directions under Step #1 to complete your DE Funding Application. Double-click on the icon for your college of interest (Chatt Tech, KSU, or GA Tech) to populate the directions.

You will remain with your regular counselor, but Dr. White and Ms. Hawk will help you develop your Dual Enrollment (DE) college schedule. They can be reached at

Please do not contact the DE counselor with your initial questions unless she is your assigned counselor. The DE counselor will reach out to you once you have completed the required form. For initial inquiries, please contact your regular counselor.
